Does Your Child Skip Words or Lines When Reading? The Link Between Eye Tracking, Ocular Motor Control, and the Vestibular System

So many parents share this exact frustration: "Whenever my kid reads a book, they're constantly skipping words or jumping lines. They'll be right in the middle of a sentence and suddenly leap to the next row. Even when I ask them to slow down and track from left to right, it's like they literally 'can't see' the words unless they guide their eyes with their finger."
If this sounds familiar, here is the first thing you need to know: this isn't necessarily an attention issue, and it definitely doesn't mean your child is just "not trying."
More often than not, the real culprit behind this struggle is weak ocular motor control and poor visual tracking. And these two visual skills are deeply intertwined with a sensory system that many people overlook—the vestibular system.
1. What Exactly are Ocular Motor Control and Eye Tracking?
To read smoothly and fluidly, our eyes need to perform three critical tasks seamlessly:
Visual Fixation: The ability to lock the eyes onto a single word to see it clearly.
Visual Tracking (Pursuits): The ability to move the eyes smoothly from left to right, following a line of text.
Saccadic Eye Movements (Saccades): The ability to accurately "jump" from the end of one line straight to the beginning of the next.
When these three foundational skills are weak, children will often exhibit noticeable red flags:
Complaining that text looks "blurry" or needing to reread sentences multiple times.
Frequently skipping words or losing their place while reading.
Moving their entire head or neck to look across a page, rather than just moving their eyes.
Getting easily fatigued, experiencing eye strain, or complaining of headaches after reading for just a short bit.
Missing text entirely when copying notes from the whiteboard at school.
2. What Does the Vestibular System Have to Do with Eye Control?
The vestibular system is located inside the inner ear and is responsible for sensing head movement, gravity, and spatial orientation. One of its most vital—yet underappreciated—jobs is keeping our eyes stable.
Think of the vestibular system like the Optical Image Stabilization (OIS) or the built-in stabilizer feature on a smartphone camera. When your head moves, your vestibular system automatically signals your eyes to make tiny, compensatory adjustments so that whatever you are looking at remains perfectly crisp and in focus. In the OT world, we call this mechanism the Vestibulo-Ocular Reflex (VOR).
This reflex is an absolute game-changer for reading because:
Our heads naturally make tiny, subtle micro-movements when we read. The vestibular system ensures eye stability so the text doesn't look like it’s "floating or bouncing around."
When shifting eyes from one line to the next, the vestibular system helps coordinate fast, pinpoint saccadic jumps to the target text.
It provides the steady, foundational sensory input needed to maintain fluid smooth pursuit movements across a page.
If a child has a sluggish or weak vestibular system, their eyes won't stabilize properly. Words will look like they are shifting or dancing on the page, naturally leading the child to skip lines just to cope.
3. Red Flags: How Can You Tell If Your Child Has Poor Vestibular-Ocular Integration?
Here are a few common warning signs to watch out for at home:
They absolutely depend on using their finger to track text; the moment they stop pointing, they lose their place or jump lines.
Their whole head moves rhythmically along with the words as they read (substituting head movement for eye movement).
They frequently miss words or copy the wrong line entirely when doing homework.
They struggle with hand-eye coordination games, like catching or throwing a ball.
They get easily carsick, crave intense spinning, or conversely, are terrified of any spinning/tilting motion.
They frequently bump into furniture or seem a bit clumsy with their overall balance.
4. How Pediatric OTs Target the Vestibular System to Supercharge Eye Tracking
Occupational therapists don't just treat the eyes in isolation. We use targeted vestibular stimulation to train the brain and eyes to communicate effectively, fixing the tracking issue right from the root source. Here are 4 ways we do this in the clinic:
🎯 Intervention 1: Therapeutic Swings (Strengthening the VOR)
What it looks like: We place the child on a specialized therapeutic swing. While they are actively swinging, we have them fixate their eyes on visual targets (like numbers or picture cards) taped to the wall, or call out words from flashcards held by the therapist.
Why it works: Because the child’s head is moving through space, the vestibular system is forced to activate its compensatory mechanisms to stabilize the eyes. This directly strengthens the Vestibulo-Ocular Reflex, making the eyes significantly more stable during static tasks like reading.
🦘 Intervention 2: Trampoline & Ball Play (Dynamic Tracking & Coordination)
What it looks like: The child bounces up and down on a trampoline while simultaneously trying to swat or catch small balls tossed by the therapist at varying heights and angles.
Why it works: Bouncing causes rapid vertical movement. The vestibular system must continuously calculate eye positioning to keep the visual field steady while tracking a moving object. This drastically boosts dynamic visual tracking and trains the eyes to lock onto target text even when the head or body experiences micro-movements.
🔦 Intervention 3: Flashlight Laser Tag (Smoothing Out Pursuits)
What it looks like: The therapist projects a fun character light (like a Minnie Mouse sticker on a flashlight beam) onto the wall or ceiling, moving it smoothly in various patterns. Keeping their head completely still, the child uses their own flashlight (e.g., Mickey Mouse) to "catch" and track the moving target using only their eyes.
Why it works: It turns a tedious eye exercise into an immersive game. It forces the child to isolate their eye movements from their head movements, directly conditioning the brain to execute smooth, horizontal, and vertical pursuit movements required for left-to-right reading tracks.
🛹 Intervention 4: Linear & Lateral Movement (Syncing the Reading Rhythm)
What it looks like: Activities involving prone scooter-board glides or side-to-side linear swinging, coupled with timing tasks where they must read symbols from left to right as they move.
Why it works: Moving laterally mirrors the left-to-right directional flow of reading. This sensory input helps the brain synchronize vestibular feedback with ocular motor tracking, establishing a smooth, natural reading rhythm.
5. Easy Sensory Games to Try at Home
You can support your child's progress with these simple, playful activities in your daily routine:
📍 Wall Trackers: Tape a horizontal row of colorful shapes or numbers on the wall from left to right. Have your child keep their head perfectly still (you can gently place your hands on their shoulders) and have their eyes jump to look at each object one by one, gradually speeding up.
🏀 Play Catch: Spend 5-10 minutes playing catch with a tennis ball or a soft beanbag. Vary the speed and distance to challenge their tracking, spatial awareness, and hand-eye coordination.
🌀 Pencil Mazes: Work on print or printable maze games together. Encourage your child to track the path with their eyes first before drawing the line with their pencil, keeping within the borders to sharpen precision visual-motor control.
6. Don't Forget About Primitive Reflexes!
Beyond the vestibular system, persistent tracking issues can also be tied to unintegrated primitive reflexes (such as the Asymmetrical Tonic Neck Reflex, or ATNR). When these infant reflexes don't fully integrate into the nervous system, they can cause a structural disconnect in eye-hand coordination. 7. When is It Time to See an Occupational Therapist?
If you notice that your child is displaying any of the following signs consistently, it is highly recommended to seek a professional evaluation:
Severe word and line skipping that doesn't improve with standard reading practice or trackers.
Frequent copying errors, omitting words, or writing completely on the wrong line.
An absolute reliance on pointing with a finger just to stay on track.
Complaints of eye fatigue, frequent headaches, or a total avoidance of reading tasks due to physical discomfort.
Noticeably poor balance, frequent tripping, or severe motion sickness.
If your child is showing these behaviors during reading or homework sessions, it's highly likely linked to underdeveloped ocular motor control, visual tracking, or vestibular system processing.
